If you've ever left a physical therapy session with faint bruise-like marks and a sudden, surprising increase in range of motion, you may have experienced a technique that uses small metal instruments to work through stubborn scar tissue and stuck fascia. The technique is called instrument-assisted soft tissue mobilization, or IASTM. What Actually Happens During Treatment

During an IASTM session, your therapist uses the tool in short, controlled strokes over the affected area, often a tendon, a scarred patch of muscle, or a stiff fascial band. Before reaching for the instrument, a good clinician will assess the area by hand first, feeling for where tissue glides normally and where it catches or resists movement. Those "stuck" spots are usually where treatment focuses.

The pressure from the instrument creates a small amount of controlled microtrauma at the tissue level. That might sound counterproductive, but it's the point: research published in the Journal of Exercise Rehabilitation describes how this process can stimulate fibroblast activity and encourage the body to lay down new, better-organized collagen, essentially prompting old, disorganized scar tissue to remodel into something more functional. Some researchers have also found that IASTM affects local temperature, circulation, and even pressure pain thresholds, which may help explain why patients sometimes feel immediate, if temporary, relief right after a session.

Patients often notice temporary redness or light bruising (called petechiae) after a session. That's a normal response, not a sign of damage, and it usually fades within a few days. Your therapist will always check in on your comfort level and adjust pressure accordingly, there's no reason IASTM should feel like something to grit your teeth through.

Where IASTM Fits Into Injury Recovery

Soft tissue injuries, sprains, strains, tendinopathies, post-surgical scarring, don't always heal in a straight line. But often they don't. Scar tissue can form in a disorganized, tangled way, gluing muscle fibers together (a process called adhesion) and limiting the glide those tissues need to move freely.

  • Post-surgical or post-injury scar tissue that's restricting movement

Some systematic reviews report meaningful improvements in pain and range of motion, particularly in the days and weeks after treatment. Others, including a 2022 meta-analysis in the British Journal of Sports Medicine — found the certainty of that evidence to be low, especially when IASTM is compared against other manual therapy techniques rather than no treatment at all. In practice, that means IASTM tends to work best as one part of a broader recovery plan, not a stand-alone fix.
